NHS announces HPV self-testing kits in a bid to eliminate cervical cancer by 2040
NHS England has announced the rollout of HPV self-testing kits to almost four million women who have not attended cervical screening after being invited to do so. The initiative forms part of the NHS’s ambition to eliminate cervical cancer by 2040. Eligible women will be able to order a free home testing kit, collect a sample themselves and return it for analysis free of charge.
The announcement comes against the backdrop of the NHS HPV vaccination programme, which has been rolled out to children in secondary school and plays a crucial role in reducing the incidence of cervical cancer among younger generations. Together, vaccination, cervical screening and now self-testing initiatives represent important steps towards the NHS’s ambition of eliminating cervical cancer as a public health concern.
Why does screening matter?
One of the greatest challenges in preventing cervical cancer is ensuring that people engage with the screening programmes available on the NHS. Almost all cases of cervical cancer are linked to high-risk strains of human papillomavirus (HPV). The HPV vaccination programme aims to protect individuals against high-risk strains of HPV that are associated with cervical cancer as well as other HPV-related cancers, and cervical screening is designed to identify HPV and abnormal cell changes before cancer develops, allowing earlier monitoring and treatment.
The screening process itself is highly effective. The challenge lies in ensuring that those who are eligible feel able to attend. NHS England reports that one third of eligible women are not up to date with their cervical screening. The reasons for this encompass a broad spectrum, including difficulties obtaining appointments, competing work or family commitments, concerns about discomfort, embarrassment and just a general lack of understanding of the importance of the screening process.
A free at-home testing kit is intended to combat some of these reasons for non-attendance and improve participation in the process.
Australia has recently been reported by the BBC to be on track to become the first country to effectively eliminate cervical cancer as a public health issue through a combination of HPV vaccination and screening programmes. Experience from countries such as Australia suggests that self-collection methods can increase participation amongst groups that have historically been under-screened, including those from minority backgrounds, disabled communities, and regional and remote communities.
Delayed diagnosis
The introduction of home HPV testing is a positive step. However, increasing screening participation alone will not eliminate the risk of delayed diagnosis.
Delayed diagnosis cases arise where abnormalities are not appropriately investigated, referrals were delayed, results were not acted upon promptly, or symptoms were not recognised despite repeated presentations to healthcare providers.
Early diagnosis can make an indisputable difference to treatment options, prognosis and long-term outcomes. Screening programmes are crucial, but patient safety also depends upon healthcare professionals recognising the warning signs and ensuring that these are investigated without unnecessary delay. This is particularly the case with cervical cancer given it can be detected at an early stage and prognosis tends to be radically improved with earlier diagnosis.
For that reason, increasing screening uptake should be viewed as one facet of a wider patient safety strategy to reduce the risk of cervical cancer.
A welcome development
The rollout of home HPV testing kits represents a significant step forward in improving access to cervical screening. By making testing free, convenient and available from home, the NHS’s hopes to encourage more women to engage with screening services and reduce the number of preventable cervical cancer diagnoses.
For patients, the message remains the same: attending screening when invited remains one of the most effective ways to reduce the risk of cervical cancer, and any initiative which makes participation easier should be welcomed.

Initiatives such as home HPV testing are a welcome development and may help remove barriers to early detection. However, screening is only the first step. Preventing avoidable harm also depends on abnormal results being acted upon promptly, symptoms being properly investigated and patients receiving timely access to further investigation and treatment when concerns arise.
