19 Jun 2012

An engineering firm have been fined £40,000 after a worker was crushed to death. Mr Mark Palmer, 46, was working on a wheeled loader, he was working under the vehicles arms when the hydraulic system lost pressure, causing the loading arm to fall and fatally crush him against the frame of the vehicle.

The court found that Hydraline Engineering Ltd had failed to complete a suitable risk assessment of the wheeled loader and had they done so the accident would have been preventable.

An investigation also found that Mr Palmer had received insufficient training for the risks associated with the hydraulic machinery and had had to learn on the job.
